Health Insurance Survey

Many members have voiced complaints about health insurance claims processing problems and with denial of coverage problems. Some have experienced excessive delays in claims processing and payments, have had to make excessive contact with Great-West claims representatives to get claims processed, and have had claims previously covered in whole or in part by our prior insurer, Allmerica, denied.

The Local is seeking information and input from the membership about their experiences. It is conceivable that the Plan's present coverage scheme is not as valuable as the prior scheme and may violate the CBA's requirement of "equivalency".

For purposes of framing a grievance or grievances, the Local would like member responses to the following survey questions:

Since Great-West has assumed coverage from Allmerica have you or any covered family members

1. Been denied coverage of a matter which was covered by Allmerica? (Please identify matter and approximate date of denial.)

2. Been paid less on a matter by Great-West than by Allmerica? (Please identify matter and approximate date.)

3. Been subject to excessive claims processing delays? How frequently?

4. Received less value in your coverage under Great-West than previously? Explain.

5. Received lasik eye surgery which was covered by Allmerica? (Identify and provide approximate date.)

Please print and mail this completed survey to Local Secretary Cynthia Tuczynski, 4355 S. Austin St., Milwaukee, WI 53207, within the next 2 weeks. Or e-mail your responses to Cindy Tuczynski. Your personal information will not be disclosed outside of the survey committee without your consent.
